- 97169 Athletic Training evaluation, low complexity
- 97170 Athletic Training evaluation, moderate complexity
- 97171 Athletic Training evaluation, high complexity

Physician or other qualified health care professional (ie, therapist) required to have direct (one-on-one) patient contact (in addition to the new codes listed above).
